Local 25 is proud to offer a Health and Wellness Center to members, in partnership with Everside Health.
The clinic, located at 1901 52nd Ave. in Moline, provides an enhanced healthcare experience to Local 25 health plan participants and their dependents. It’s an easier, more convenient option for everything from preventive screenings to chronic condition management, to mental health and more. The best part: It’s free. No co-pays, no deductibles and no co-insurance to health plan participants.
It’s easy to message or meet with a primary care provider using the Everside Health app, available for free download in the Apple App Store or Google Play Store. Appointments can also be scheduled directly through the app or by calling 309-324-4515. Members can also visit members.eversidehealth.com and click on “Sign up” to start an account.

Who is eligible?
All members and plan participants covered by the Local Union 25 Health & Welfare plan are eligible to receive Everside services.
Do I have to make Everside my primary care provider to use the services?
No. Everside is an optional benefit that functions as an add-on to your existing health plan. You don’t need to switch providers or stop seeing a preferred specialist — you can see Everside providers as well as your existing provider.
Does using Everside services increase my healthcare costs?
No, it should do just the opposite. By visiting the Local 25 Health & Wellness Center, you’re saving yourself an expensive trip to the emergency room or urgent care, and visits are at NO COST for preventive and non-preventive services. There may also be some tests and procedures billed to your primary insurance at rates much lower than with other providers.
